The access and management of clinical data, controlled by the patient with the help of his referral doctor, has to be driven by the medical profession. Medical secrecy and the ways and customs of medical providers have to be respected when sharing medical data.
CRISNET (www.crisnet.be) is a non profit association created by a group of French speaking Belgian GPs that has developed an open source patient oriented internet based medical record.
The combination of our experiences from our medical centres and the expertise in security of our computer specialist form the basis of a step-by-step development of our internet based electronic patient record. Previously, in order for a data oriented and coded problem (ICPC -ICD) to be shared, it would have to be structured by episode, clear hierarchies organised the access of specific providers and the referral doctor managed the coordination between various actors.
With OPENCARE, information such as data from labs, administrative and medical documents and hospital based specialist reports are now easily accessible. A secure identification and third party billing system is operational. Professional, legal and ethical recommendations are implemented. The whole system has a high level of security and patient controlled confidentiality through an original mandate system.
Parts of this system are already in use in a Belgian GP based sentinel network, through an agreement between SSMG (Belgian French speaking society of GP/FM) and RESOPRIM, a university based research network.
An online presentation of a prototype of OPENCARE will allow us to share our main problems and solutions with European GPs and to show that the development of the highly sophisticated system is possible without huge financial investments.
